To recognize annually, an individual who is an active member of the AACPDM and who has demonstrated outstanding leadership mentoring trainees and colleagues in the field of childhood-onset disabilities.
Eligibility: Both nominator and nominee must be current members of the AACPDM.
Nominee’s exceptional achievement in mentoring trainees and colleagues in the field of childhood-onset disabilities will be considered. This will include a focus on the sustainability of mentorship over time and the scope/depth of the mentorship.
Nominations are due in the AACPDM office by 5:00 p.m. CST on May 31st of the current year, and may be electronically submitted to email@example.com. All nominations are confidential. The nomination package should consist of a nomination letter, current curriculum vitae of the nominee, and two additional support letters. (Publications/posters by nominators should be highlighted/bolded in the nominee’s CV.)
The nomination letter should be limited to no more than 2 single-spaced pages and should include:
Two additional support letters (also no more than 2 single-spaced pages each) should be included from other individuals that attest to the quality and impact of the nominee's mentorship.
The Research Committee will review the nominations and recommend an award recipient to the Board of the AACPDM for final approval. Once Board approval is obtained, the recipient and nominator will be notified of the award in writing by June 30th of the current year. A public announcement and presentation of the award will be made at the AACPDM annual conference. The award will consist of a plaque.
