The Care and Outreach Coordinator has an overall responsibility and accountability for coordinating all aspects of the individuals care, including but not limited to health and mental healthcare, community supports, and other services required to meet the needs of the individual. Comprehensive Care Management.

Position Responsibilities
• Triage new and existing patients, complete psychosocial assessment using person- centered planning techniques, as well as gathers and incorporates all other relevant assessments.
• Meets with individuals in the provider offices, and other public places in order to conduct assessments and provide services.
• Develops a comprehensive plan of care with multidisciplinary team for higher level of functioning.
• Reviews the care plan with the individual’s entire interdisciplinary care team. Collaborates with providers and service support team members
• Advocates for patients and families by communicating their needs to other disciplines and helping to navigate the medical system.
• Provide and follow through on referrals to appropriate community resources (transportation, WC clinic, home health, PT, early intervention, adaptive resource, Medicaid, OPWDD, housing etc)
• Participate in ongoing short and learn term projects- supervising SW intern (if applicable), updating resource list, planning wellness events, and outreach to community partners about The Center.
• Supports the individual in the planning process to ensure that the individual directs the process to the maximum extent possible and can make informed decisions and choices.
• Effectively manage a tiered caseload, while tailoring services to individual needs.
• Assist with completion of patient consents and authorizations to share information.
• Develops and maintains patient documentation in Epic
• Completes documentation and billing in a timely manner.
• Monitors individual satisfaction with supports and services.
• Provide quality driven, cost effective, culturally appropriate services.
• Knowledge of developmental disabilities, chronic disease and social determinants of health.
• Ability to build relationships and effectively communicate.
• Collaborate with other internal departments and external community-based organizations and providers, as needed. Internal care partners include the core referral Departments (Rehab, Neuro, GI, NICU, and complex care team) that care for many of outpatients as well as other division in Orthopedics and other Departments.
• Acts as representative to the CP Center network; attend monthly Teams meetings, contribute to the design and planning of activities to educate the community about Cerebral Palsy
• Coordinates and Manages CP Center Sponsorships and coordinates essential CP Center initiative logistics (team meeting agenda, zoom set up etc.).

Minimum Qualifications
Bachelor’s degree with 2 years relevant experience, required,
OR A Licensed Registered Nurse or other clinical training with relevant experience,
OR
A LMSW degree with relevant experience
