AACPDM Mentorship Award

Purpose
To recognize annually, an individual who is an active member of the AACPDM and who has demonstrated outstanding leadership mentoring trainees and colleagues in the field of childhood-onset disabilities.

Eligibility
Both nominator and nominee must be current members of the AACPDM.

Selection Criteria
Nominee's exceptional achievement in mentoring trainees and colleagues in the field of childhood-onset disabilities will be considered, with particular emphasis on a history of sustained mentorship over time and the scope/depth of the mentorship.

Nomination Procedure
Step 1: Submit electronically the name, title and current position and contact information of the nominee, and a brief (100 word) statement in support of the nomination.

Step 1 submission: The nomination letter should be limited to no more than 2 single-spaced pages and should include:

• A clear and full description of the nature of the nominee's exceptional achievement in mentoring, with a focus on the contribution and impact of the mentoring to improving services/care, promoting professional education and/or promoting research for children with disabilities.
• Name and relationship of the nominator to the nominee
• The nominator's contact information - TITLE, DEGREE(S), INSTITUTION, ADDRESS, PHONE NUMBER, EMAIL ADDRESS
• All nominations are confidential. The nomination package should consist of a nomination letter, current curriculum vitae of the nominee, and at least two additional letters of support. (Publications/presentations/posters by nominators should be highlighted/bolded in the nominee's CV.)

Deadline for (Step 1) nominee name-only submission to mentorship@aacpdm.org is 5:00 p.m. CST February 15, 2012.

Step 2: The research committee will select a short list from the submissions and invite the nominators to submit the full nomination package.

At least two additional support letters (also no more than 2 single-spaced pages each) should be included from other individuals that attest to the quality and impact of the nominee's mentorship.

Deadline for the electronic submission of full nomination package (Step 2) to mentorship@aacpdm.org is 5:00 p.m. CST on April 1, 2012.

Selection and notification
The Research Committee will review the nominations and forward their recommendation to the Board of Directors for final approval. Once Board approval is obtained, the recipient and nominator will be notified in writing. A public announcement and presentation of the award will be made at the AACPDM Annual Meeting.
